cpufreq: fix garbage kobjects on errors during suspend/resume

This is effectively a revert of commit 5302c3fb2e62 ("cpufreq: Perform
light-weight init/teardown during suspend/resume"), which enabled
suspend/resume optimizations leaving the sysfs files in place.

Errors during suspend/resume are not handled properly, leaving
dead sysfs attributes in case of failures.  There are are number of
functions with special code for the "frozen" case, and all these
need to also have special error handling.

The problem is easy to demonstrate by making cpufreq_driver->init()
or cpufreq_driver->get() fail during resume.

The code is too complex for a simple fix, with split code paths
in multiple blocks within a number of functions.  It is therefore
best to revert the patch enabling this code until the error handling
is in place.

The failure to restore cpufreq devices on cancelled hibernation is
not a new bug. It is caused by the ACPI _PPC call failing unless the
hibernate is completed. This makes the acpi_cpufreq driver fail its
init.

Previously, the cpufreq device could be restored by offlining the
cpu temporarily.  And as a complete hibernation cycle would do this,
it would be automatically restored most of the time.  But after
commit 5302c3fb2e62 the leftover sysfs attributes will block any
device add action.  Therefore offlining and onlining CPU 1 will no
longer restore the cpufreq object, and a complete suspend/resume
cycle will replace it with garbage.

cpufreq: suspend governors on system suspend/hibernate

This patch adds cpufreq suspend/resume calls to dpm_{suspend|resume}_noirq()
for handling suspend/resume of cpufreq governors.

Lan Tianyu (Intel) & Jinhyuk Choi (Broadcom) found anr issue where
tunables configuration for clusters/sockets with non-boot CPUs was
getting lost after suspend/resume, as we were notifying governors
with CPUFREQ_GOV_POLICY_EXIT on removal of the last cpu for that
policy and so deallocating memory for tunables. This is fixed by
this patch as we don't allow any operation on governors after
device suspend and before device resume now.

cpufreq: tegra: don't error target() when suspended

d4019f0a92ab "cpufreq: move freq change notifications to cpufreq core"
added code to the cpufreq core to print an error if a cpufreq driver's
.target() function returned an error. This exposed the fact that Tegra's
cpufreq driver returns an error when it is ignoring requests due to the
system being suspended.

Modify Tegra's .target() function not to return an error in this case;
this prevents the error prints. The argument is that since the suspend
hook can't and doesn't inform the cpufreq core when its requests will
be ignored, there's no way for the cpufreq core to squelch them, so it's
not an error for the requests to keep coming. This change make the Tegra
driver consistent with how the Exynos handles the same situation. Note
that s5pv210-cpufreq.c probably suffers from this same issue though.

When decreasing frequency, requested_freq may be less than
freq_target, So requested_freq minus freq_target may be negative,
But reqested_freq's unit is unsigned int, then the negative result
will be one larger interger which may be even higher than
requested_freq.

This patch is to fix such issue. when result becomes negative,
set requested_freq as the min value of policy.

intel_pstate: skip the driver if ACPI has power mgmt option

Do not load the Intel pstate driver if the platform firmware
(ACPI BIOS) supports the power management alternatives.
The ACPI BIOS indicates that the OS control mode can be used
if the _PSS (Performance Supported States) is defined in ACPI
table. For the OS control mode, the Intel pstate driver will be
loaded.

HP BIOS has several power management modes (firmware, OS-control and
so on). For the OS control mode in HP BIOS, the Intel p-state driver
will be loaded. When the customer chooses the firmware power
management in HP BIOS, the Intel p-state driver will be ignored.

I put hw_vendor_info vendor_info in case other vendors (Dell, Lenovo...)
have their firmware power management. Vendors should make sure their
firmware power management works properly, and they can go for adding
their vendor info to the variable.

I have verified the patch on HP ProLiant servers.  The patch worked
correctly.

behaving correctly.  When the BIOS is configured for OS control for
power management, the intel_pstate driver loads as expected.  When

intel_pstate driver does not load and we get the pcc_cpufreq driver
instead.]

Most of the drivers do following in their ->target_index() routines:

cpufreq_notify_transition(policy, &freqs, CPUFREQ_POSTCHANGE);

This is replicated over all cpufreq drivers today and there doesn't exists a
good enough reason why this shouldn't be moved to cpufreq core instead.

There are few special cases though, like exynos5440, which doesn't do everything
on the call to ->target_index() routine and call some kind of bottom halves for
doing this work, work/tasklet/etc..

They may continue doing notification from their own code as flag:
CPUFREQ_ASYNC_NOTIFICATION is already set for them.

All drivers are also modified in this patch to avoid breaking 'git bisect', as
double notification would happen otherwise.

There are few special cases like exynos5440 which doesn't send POSTCHANGE
notification from their ->target() routine and call some kind of bottom halves
for doing this work, work/tasklet/etc.. From which they finally send POSTCHANGE
notification.

Its better if we distinguish them from other cpufreq drivers in some way so that
core can handle them specially. So this patch introduces another flag:
CPUFREQ_ASYNC_NOTIFICATION, which will be set by such drivers.

This also changes exynos5440-cpufreq.c and powernow-k8 in order to set this
flag.

The b.L switcher can be turned on/off at run time.  It is therefore
necessary to change the cpufreq driver behavior accordingly.

The driver must be unregistered/registered with the cpufreq core
to reconfigure freq tables for the virtual or actual CPUs. This is
accomplished via the b.L switcher notifier callback.

This patch adds IKS (In Kernel Switcher) support to cpufreq driver.

This creates a combined freq table for A7-A15 CPU pairs. A7 frequencies
are virtualized and scaled down to half the actual frequencies to
approximate a linear scale across the combined A7+A15 range. When the
requested frequency change crosses the A7-A15 boundary a cluster switch
is invoked.

Based on earlier work from Sudeep KarkadaNagesha.

On TC2, the cpu clocks are controlled by the external M3 microcontroller
and SPC provides the interface between the CPU and the power controller.

The generic cpufreq drivers use the clock APIs to get the cpu clocks.
This patch add virtual spc clocks for all the cpus to control the cpu
operating frequency via the clock framework.

ARM: vexpress/TC2: add support for CPU DVFS

SPC(Serial Power Controller) on TC2 also controls the CPU performance
operating points which is essential to provide CPU DVFS. The M3
microcontroller provides two sets of eight performance values, one set
for each cluster (CA15 or CA7). Each of this value contains the
frequency(kHz) and voltage(mV) at that performance level. It expects
these performance level to be passed through the SPC PERF_LVL registers.

This patch adds support to populate these performance levels from M3,
build the mapping to CPU OPPs at the boot and then use it to get and
set the CPU performance level runtime.

We have per-CPU cpu_policy_rwsem for cpufreq core, but we never use
all of them. We always use rwsem of policy->cpu and so we can
actually make this rwsem per policy instead.

This patch does this change. With this change other tricky situations
are also avoided now, like which lock to take while we are changing
policy->cpu, etc.

cpufreq: Implement light weight ->target_index() routine

Currently, the prototype of cpufreq_drivers target routines is:

int target(struct cpufreq_policy *policy, unsigned int target_freq,
unsigned int relation);

And most of the drivers call cpufreq_frequency_table_target() to get a valid
index of their frequency table which is closest to the target_freq. And they
don't use target_freq and relation after that.

So, it makes sense to just do this work in cpufreq core before calling
cpufreq_frequency_table_target() and simply pass index instead. But this can be
done only with drivers which expose their frequency table with cpufreq core. For
others we need to stick with the old prototype of target() until those drivers
are converted to expose frequency tables.

This patch implements the new light weight prototype for target_index() routine.
It looks like this:

int target_index(struct cpufreq_policy *policy, unsigned int index);

CPUFreq core will call cpufreq_frequency_table_target() before calling this
routine and pass index to it. Because CPUFreq core now requires to call routines
present in freq_table.c CONFIG_CPU_FREQ_TABLE must be enabled all the time.

This also marks target() interface as deprecated. So, that new drivers avoid
using it. And Documentation is updated accordingly.

It also converts existing .target() to newly defined light weight
.target_index() routine for many driver.

changed the conditional around the declaration of usb_nop_xceiv_register
from
#if defined(CONFIG_NOP_USB_XCEIV) ||
(defined(CONFIG_NOP_USB_XCEIV_MODULE) && defined(MODULE))
to
#if IS_ENABLED(CONFIG_NOP_USB_XCEIV)

While that looks the same, it is semantically different. The first expression
is true if CONFIG_NOP_USB_XCEIV is built as module and if the including
code is built as module. The second expression is true if code depending on
CONFIG_NOP_USB_XCEIV if built as module or into the kernel.

As a result, the arm:allmodconfig build fails with

arch/arm/mach-omap2/built-in.o: In function `omap3_evm_init':
arch/arm/mach-omap2/board-omap3evm.c:703: undefined reference to
`usb_nop_xceiv_register'

Fix the problem by reverting to the old conditional.

ACPI / PM: Drop two functions that are not used any more

Two functions defined in device_pm.c, acpi_dev_pm_add_dependent()
and acpi_dev_pm_remove_dependent(), have no callers and may be
dropped, so drop them.

Moreover, they are the only functions adding entries to and removing
entries from the power_dependent list in struct acpi_device, so drop
that list too.

ATA / ACPI: remove power dependent device handling

Previously, we wanted SCSI devices corrsponding to ATA devices to
be runtime resumed when the power resource for those ATA device was
turned on by some other device, so we added the SCSI device to the
dependent device list of the ATA device's ACPI node.  However, this
code has no effect after commit 41863fc (ACPI / power: Drop automaitc
resume of power resource dependent devices) and the mechanism it was
supposed to implement is regarded as a bad idea now, so drop it.

direct register manipulation.

Such approach is not allowed in the common clock framework. The frequency
is changed, but the corresponding clock value is not updated. This causes
wrong frequency read from cpufreq's cpuinfo_cur_freq sysfs attribute.

Also direct manipulation with PLL's S parameter has been removed. It is
already done at PLL35xx code.

Revert commit 1ecfd533f4c5 ("mm/mremap.c: call pud_free() after fail
calling pmd_alloc()").

The original code was correct: pud_alloc(), pmd_alloc(), pte_alloc_map()
ensure that the pud, pmd, pt is already allocated, and seldom do they
need to allocate; on failure, upper levels are freed if appropriate by
the subsequent do_munmap().  Whereas commit 1ecfd533f4c5 did an
unconditional pud_free() of a most-likely still-in-use pud: saved only
by the near-impossiblity of pmd_alloc() failing.

Occasionally we hit the BUG_ON(pmd_trans_huge(*pmd)) at the end of
__split_huge_page_pmd(): seen when doing madvise(,,MADV_DONTNEED).

It's invalid: we don't always have down_write of mmap_sem there: a racing
do_huge_pmd_wp_page() might have copied-on-write to another huge page

swap: fix set_blocksize race during swapon/swapoff

Fix race between swapoff and swapon.  Swapoff used old_block_size from
swap_info outside of swapon_mutex so it could be overwritten by
concurrent swapon.

The race has visible effect only if more than one swap block device
exists with different block sizes (e.g.  /dev/sda1 with block size 4096
and /dev/sdb1 with 512).  In such case it leads to setting the blocksize
of swapped off device with wrong blocksize.

The bug can be triggered with multiple concurrent swapoff and swapon:
0. Swap for some device is on.
1. swapoff:
First the swapoff is called on this device and "struct swap_info_struct
*p" is assigned. This is done under swap_lock however this lock is
released for the call try_to_unuse().

2. swapon:
After the assignment above (and before acquiring swapon_mutex &
swap_lock by swapoff) the swapon is called on the same device.
The p->old_block_size is assigned to the value of block_size the device.
This block size should be the same as previous but sometimes it is not.
The swapon ends successfully.

3. swapoff:
Swapoff resumes, grabs the locks and mutex and continues to disable this
swap device. Now it sets the block size to value taken from swap_info
which was overwritten by swapon in 2.

Commit c4fe24485729 ("sparc: fix PCI device proc file mmap(2)") added
proc_reg_get_unmapped_area in proc_reg_file_ops and
proc_reg_file_ops_no_compat, by which now mmap always returns EIO if
get_unmapped_area method is not defined for the target procfs file,
which causes regression of mmap on /proc/vmcore.

To address this issue, like get_unmapped_area(), call default
current->mm->get_unmapped_area on MMU-present architectures if
pde->proc_fops->get_unmapped_area, i.e.  the one in actual file
operation in the procfs file, is not defined.

Currently, proc_reg_get_unmapped_area truncates upper 32-bit of the
mapped virtual address returned from get_unmapped_area method in
pde->proc_fops due to the variable rv of signed integer on x86_64.  This
is too small to have vitual address of unsigned long on x86_64 since on
x86_64, signed integer is of 4 bytes while unsigned long is of 8 bytes.
To fix this issue, use unsigned long instead.

Fixes a regression added in commit c4fe24485729 ("sparc: fix PCI device
proc file mmap(2)").

Since pause is bounded by [min_pause, max_pause] where min_pause is also
bounded by max_pause.  It's suspected and demonstrated that the
max_pause calculation goes wrong:

The problem lies in the two "long = unsigned long" assignments in
bdi_max_pause() which might go negative if the highest bit is 1, and the

fs: buffer: move allocation failure loop into the allocator

Buffer allocation has a very crude indefinite loop around waking the
flusher threads and performing global NOFS direct reclaim because it can
not handle allocation failures.

The most immediate problem with this is that the allocation may fail due
to a memory cgroup limit, where flushers + direct reclaim might not make
any progress towards resolving the situation at all.  Because unlike the
global case, a memory cgroup may not have any cache at all, only
anonymous pages but no swap.  This situation will lead to a reclaim
livelock with insane IO from waking the flushers and thrashing unrelated
filesystem cache in a tight loop.

Use __GFP_NOFAIL allocations for buffers for now.  This makes sure that
any looping happens in the page allocator, which knows how to
orchestrate kswapd, direct reclaim, and the flushers sensibly.  It also
allows memory cgroups to detect allocations that can't handle failure
and will allow them to ultimately bypass the limit if reclaim can not
make progress.

Commit 3812c8c8f395 ("mm: memcg: do not trap chargers with full
callstack on OOM") assumed that only a few places that can trigger a
memcg OOM situation do not return VM_FAULT_OOM, like optional page cache
readahead.  But there are many more and it's impractical to annotate
them all.

First of all, we don't want to invoke the OOM killer when the failed
allocation is gracefully handled, so defer the actual kill to the end of
the fault handling as well.  This simplifies the code quite a bit for
added bonus.

Second, since a failed allocation might not be the abrupt end of the
fault, the memcg OOM handler needs to be re-entrant until the fault
finishes for subsequent allocation attempts.  If an allocation is
attempted after the task already OOMed, allow it to bypass the limit so
that it can quickly finish the fault and invoke the OOM killer.

In commit 27a7c642174e ("partitions/efi: account for pmbr size in lba")
we started treating bad sizes in lba field of the partition that has the
0xEE (GPT protective) as errors.

However, we may run into these "bad sizes" in the real world if someone
uses dd to copy an image from a smaller disk to a bigger disk.  Since
this case used to work (even without using force_gpt), keep it working
and treat the size mismatch as a warning instead of an error.

Commit 11feeb498086 ("kvm: optimize away THP checks in
kvm_is_mmio_pfn()") introduced a memory leak when KVM is run on gigantic
compound pages.

That commit depends on the assumption that PG_reserved is identical for
all head and tail pages of a compound page.  So that if get_user_pages
returns a tail page, we don't need to check the head page in order to
know if we deal with a reserved page that requires different
refcounting.

The assumption that PG_reserved is the same for head and tail pages is
certainly correct for THP and regular hugepages, but gigantic hugepages
allocated through bootmem don't clear the PG_reserved on the tail pages
(the clearing of PG_reserved is done later only if the gigantic hugepage
is freed).

This patch corrects the gigantic compound page initialization so that we
can retain the optimization in 11feeb498086.  The cacheline was already
modified in order to set PG_tail so this won't affect the boot time of
large memory systems.

zswap_writeback_entry do nothing except put refcount
Now, the memory of zswap_entry x and its zpage leak.

Modify:
 - check the refcount in fail path, free memory if it is not referenced.

 - use ZSWAP_SWAPCACHE_FAIL instead of ZSWAP_SWAPCACHE_NOMEM as the fail path
   can be not only caused by nomem but also by invalidate.

We should clear the page's private flag when returing the page to the
hugepage pool.  Otherwise, marked hugepage can be allocated to the user
who tries to allocate the non-reserved hugepage.  If this user fail to
map this hugepage, he would try to return the page to the hugepage pool.
Since this page has a private flag, resv_huge_pages would mistakenly
increase.  This patch fixes this situation.

After acquiring the semlock spinlock, operations must test that the
array is still valid.

 - semctl() and exit_sem() would walk stale linked lists (ugly, but
   should be ok: all lists are empty)

 - semtimedop() would sleep forever - and if woken up due to a signal -
   access memory after free.

The patch also:
 - standardizes the tests for .deleted, so that all tests in one
   function leave the function with the same approach.
 - unconditionally tests for .deleted immediately after every call to
   sem_lock - even it it means that for semctl(GETALL), .deleted will be
   tested twice.

Both changes make the review simpler: After every sem_lock, there must
be a test of .deleted, followed by a goto to the cleanup code (if the
function uses "goto cleanup").

The only exception is semctl_down(): If sem_ids().rwsem is locked, then
the presence in ids->ipcs_idr is equivalent to !.deleted, thus no
additional test is required.

mm, memcg: protect mem_cgroup_read_events for cpu hotplug

for_each_online_cpu() needs the protection of {get,put}_online_cpus() so
cpu_online_mask doesn't change during the iteration.

cpu_hotplug.lock is held while a cpu is going down, it's a coarse lock
that is used kernel-wide to synchronize cpu hotplug activity.  Memcg has
a cpu hotplug notifier, called while there may not be any cpu hotplug
refcounts, which drains per-cpu event counts to memcg->nocpu_base.events
to maintain a cumulative event count as cpus disappear.  Without
get_online_cpus() in mem_cgroup_read_events(), it's possible to account
for the event count on a dying cpu twice, and this value may be
significantly large.

In fact, all memcg->pcp_counter_lock use should be nested by
{get,put}_online_cpus().

This fixes that issue and ensures the reported statistics are not vastly
over-reported during cpu hotplug.

The function update_policy_cpu() is expected to be called when the policy->cpu
of a cpufreq policy is to be changed: ie., the new CPU nominated to become the
policy->cpu is different from the old one.

Print a warning if it is invoked with new_cpu == old_cpu, since such an
invocation might hint at a faulty logic in the caller.

Some functions on switch path use msleep() which is inaccurate, and
depends on HZ. With HZ=100 msleep(1) takes actually over ten times longer.
Using usleep_range() we get more accurate sleeps.

I measured the "pfunc_slewing_done" polling to take 300us at max (on
2.3GHz dual-processor Xserve G5), so using 500us sleep there should
be fine.

With the patch, g5_switch_freq() duration drops from ~50ms to ~10ms on
Xserve with HZ=100.

so drivers aren't required to set it. When space_id is ACPI_ADR_SPACE_SYSTEM_IO
for acpi cpufreq driver it doesn't set ->get to a valid function pointer and so
policy->cur is required to be set by driver.

This is already followed in acpi-cpufreq driver. This patch adds a comment
describing why we need to set policy->cur from driver.

Currently clk_get_sys is used with cpu-cluster.<n> as the device id
which is incorrect. It should be connection/consumer ID instead.

It is possible to specify input clock in the cpu device node along
with the optional clock-name. clk_get_sys can't handle that.

This patch replaces clk_get_sys with clk_get to extend support for
clocks specified in the device tree cpu node.

ACPI / power: Drop automaitc resume of power resource dependent devices

The mechanism causing devices depending on a given power resource
(that is, devices that can be in D0 only if that power resource is
on) to be resumed automatically when the power resource is turned
on (and their "inferred" power state becomes D0 as a result) is
inherently racy and in fact unnecessary.

It is racy, because if the power resource is turned on and then
immediately off, the device resume triggered by the first transition
to "on" may still happen, causing the power resource to be turned
on again.  That again will trigger the "resume of dependent devices"
mechanism, but if the devices in question are not in use, they will
be suspended in the meantime causing the power resource to be turned
off.  However, the "resume of dependent devices" will next resume
them again and so on.  In some cases (USB port PM in particular) that
leads to an endless busy loop of flipping the resource on and off
continuously.

It is needless, because whoever turns a power resource on will most
likely turn it off at some point and the devices that go into "D0"
as a result of turning it on will then go back into D3cold
(generally, the state they were in before).

Moreover, turning on all power resources a device needs to go into
D0 is not sufficient for a full transition into D0 in general.
Namely, _PS0 may need to be executed in addition to that in some
cases.  This means that the whole rationale of the "resume of
dependent devices" mechanism was incorrect to begin with and it's
best to remove it entirely.

warning to be emitted:

which happens because unsigned long is 32-bit on some architectures.

Fix that by using a helper u64 variable and simplify the code
slightly.

Some USB drive enclosures do not correctly report an
overflow condition if they hold a drive with a capacity
over 2TB and are confronted with a READ_CAPACITY_10.
They answer with their capacity modulo 2TB.
The generic layer cannot cope with that. It must be told
to use READ_CAPACITY_16 from the beginning.

This patch fixes a particular type of data corruption that has been
encountered when loading a snapshot's metadata from disk.

When we allocate a new chunk in persistent_prepare, we increment
ps->next_free and we make sure that it doesn't point to a metadata area
by further incrementing it if necessary.

When we load metadata from disk on device activation, ps->next_free is
positioned after the last used data chunk. However, if this last used
data chunk is followed by a metadata area, ps->next_free is positioned
erroneously to the metadata area. A newly-allocated chunk is placed at
the same location as the metadata area, resulting in data or metadata
corruption.

This patch changes the code so that ps->next_free skips the metadata
area when metadata are loaded in function read_exceptions.

The patch also moves a piece of code from persistent_prepare_exception
to a separate function skip_metadata to avoid code duplication.

The profile introspection seq file has a locking bug when policy is viewed
from a virtual root (task in a policy namespace), introspection from the
real root is not affected.

The test for root
    while (parent) {
is correct for the real root, but incorrect for tasks in a policy namespace.
This allows the task to walk backup the policy tree past its virtual root
causing it to be unlocked before the virtual root should be in the p_stop
fn.

  The bug fixes removes a scary but harmless warning on SPARC that was
