percpu: stop leaking bitmap metadata blocks

The commit ca460b3c9627 ("percpu: introduce bitmap metadata blocks")
introduced bitmap metadata blocks. These metadata blocks are allocated
whenever a new chunk is created, but they are never freed. Fix it.
